Syringe pumps are primarily used in the neonatal and pediatric populations or in operating rooms and intensive care units for the adult population.

  Smiths Medical is recalling specific software versions of the Medfusion 3500 and 4000 Syringe Pumps because of a software error that may lead to over-delivery or under-delivery of fluids or medication. Over- or under-delivery can occur if the following specific sequence of events occur: a bolus or loading dose is interrupted, the pump is primed, and the infusion is restarted. Use of the affected syringe pumps may cause serious adverse health consequences including death.

  There have been four complaints regarding this device issue including one serious injury. There have been no deaths.

• Faribault Foods Inc., a Faribault, Minn. establishment, is recalling approximately 15,134 pounds of canned soup product due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The product contains milk and soy, known allergens, as well as beef and pork, which are not declared on the product label.

  The cans labeled as chicken noodle soup actually contain a meatball and pasta product. The canned soup items were produced on May 26, 2020. The following products are subject to recall: [View Labels (PDF only)]

• Fred Meyer is recalling cheese dips sold in our delis because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. • The U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) is issuing a public health alert because Bluegrass Provisions Co., a Crescent Springs, Ky. establishment, produced sausage products that may be contaminated with Listeria monocytogenes.   The products bear establishment number “EST. 7417” inside the USDA mark of inspection. These items were shipped to distributors and retail locations in Kentucky, Ohio, North Carolina, and Virginia.

  The problem was discovered by routine testing and the results showed one of the products was contaminated with Listeria monocytogenes. The additional products may be affected by cross-contamination.

  Consumption of food contaminated with L. monocytogenes can cause listeriosis, a serious infection that primarily affects older adults, persons with weakened immune systems, and pregnant women and their newborns. Less commonly, persons outside these risk groups are affected.

  FSIS is concerned that some product may be in consumers’ freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

  FSIS advises all consumers to reheat ready-to-eat product until steaming hot.

• Ferring Pharmaceuticals US is voluntarily recalling all lots on the market of DDAVP® Nasal Spray 10 mcg/0.1mL, Desmopressin Acetate Nasal Spray 10 mcg/0.1mL, and STIMATE® Nasal Spray 1.5 mg/mL listed in the table below to the consumer level. These products are being recalled due to super potency or amounts of desmopressin higher than specified. These out of specification results were obtained during routine testing.The risks associated with higher than specified amounts of desmopressin relate to abnormally low levels of sodium in the blood (i.e., hyponatremia) which could eventually lead to seizure, coma, and death. To date, Ferring has not received an increase in adverse event reports due to increased concentrations of desmopressin from users of the nasal spray. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. The problem was discovered by Ajinomoto Foods North America, Inc. when they identified pieces of hard plastic in their production process and in a barrel of diced green chilies that was received from their ingredients supplier, Sun Valley Foods. Sun Valley Foods initiated a recall of the green chilies with the FDA. As more FDA information becomes available, FSIS will update this public health alert.
