Following an 1861 decision by a Union general, escaped slaves were declared contraband, or illegal war goods, and freed. Thousands of fugitive slaves, including this group in Pamunkey Run, Virginia, provided the Union army with labor and established independent communities.

  • Date Initiated by Firm: June 26, 2020The Smiths Medical Medfusion Syringe Pump delivers blood, blood products, prescribed drugs, and other fluids into a patient’s body in a controlled manner. Syringe pumps are primarily used in the neonatal and pediatric populations or in operating rooms and intensive care units for the adult population.

    Smiths Medical is recalling specific software versions of the Medfusion 3500 and 4000 Syringe Pumps because of a software error that may lead to over-delivery or under-delivery of fluids or medication. Over- or under-delivery can occur if the following specific sequence of events occur: a bolus or loading dose is interrupted, the pump is primed, and the infusion is restarted. Use of the affected syringe pumps may cause serious adverse health consequences including death.

    There have been four complaints regarding this device issue including one serious injury. There have been no deaths.

      • Locate all Medfusion® 3500 and 4000 Syringe Pumps in possession and verify the firmware version to determine if the pump(s) is impacted by this notice.
      • Ensure all personnel who may utilize the pump are aware of this recall.
      • For any reason, when a bolus or loading dose delivery is interrupted, cancel the bolus or loading dose and begin a new infusion to avoid potential inappropriate delivery.
      • Complete and return the response form attached to the notice and email it to Smiths Medical at fieldactions@smiths-medical.com within 10 days of receipt.
        • Distributors share the notification with customers who have received the affected product and ask each customer to complete the response form contained in the notice and return to back to the distributor.

  • Sunshine Mills, Inc. is issuing a voluntary recall of Nature’s Menu® Super Premium Dog Food with a Blend of Real Chicken & Quail because of the potential to be contaminated with Salmonella. Salmonella can affect animals eating the products and there is risk to humans from handling contaminated products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products.Healthy people infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. Rarely, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans.     No illnesses, injuries or complaints have been reported to date.

    The potential for contamination was noted after the firm was notified by the Georgia Department of Agriculture when a sample of a single 3-pound bag of the product was collected and tested positive for Salmonella.  The firm conducted testing of multiple samples of the same lot which all tested negative for SalmonellaSalmonellamay not be evenly distributed throughout a lot which is why it could have been found on one sample and not on multiple other samples of the same lot.  However, out of an abundance of caution, Sunshine Mills, Inc. has chosen to issue a voluntary recall of the above-referenced lots of Nature’s Menu® Super Premium Dog Food with a Blend of Real Chicken & Quail in furtherance of its commitment to the safety and quality of its products.

    Retailers who received the recalled lots have been contacted and asked to pull these lots from their inventory and shelves.  Consumers who have purchased Nature’s Menu® Super Premium Dog Food with a Blend of Real Chicken & Quail from the recalled lots should discontinue use of the product and may return the unused portion to the place of purchase for a full refund.   • Invokana, Invokamet, Invokamet XR (canagliflozin): MedWatch Safety Alert – Boxed Warning about Risk of Leg and Foot Amputations Removed  Patient, Health Professional, Endocrinology, PharmacyISSUE: Based on FDA’s review of new data from three clinical trials, the boxed warning about amputation risk from the diabetes medicine canagliflozin (Invokana, Invokamet, Invokamet XR) was removed from the prescribing information.FDA reviews of new clinical trial data demonstrated additional heart- and kidney-related benefits, which led to additional approved uses.  Specifically, in 2018, canagliflozin was approved to reduce the risk of major heart-related events such as heart attack, stroke, or death in patients with type 2 diabetes who have known heart disease; and, in 2019, it was approved to reduce the risk of end-stage kidney disease, worsening of kidney function, heart-related death, and being hospitalized for heart failure in certain patients with type 2 diabetes and diabetic kidney disease.Collectively, these newly identified effects of canagliflozin on heart and kidney disease show significantly enhanced benefit of this medicine.  Safety information from recent clinical trials also suggests that the risk of amputation, while still increased with canagliflozin, is lower than previously described, particularly when appropriately monitored.  Based upon these considerations, FDA has concluded that the boxed warning should be removed.BACKGROUND: Today’s information is an update to the FDA Drug Safety Communication: FDA confirms increased risk of leg and foot amputations with the diabetes medicine canagliflozin (Invokana, Invokamet, Invokamet XR) issued on May 16, 2017.Canagliflozin belongs to a class of medicines called sodium-glucose cotransporter-2 (SGLT2) inhibitors.  It lowers blood sugar by causing the kidneys to remove sugar from the body through the urine.

FDA laboratory analysis found ZANILAST+ GEL to contain 1-propanol.Risk Statement: There is a reasonable probability of acute toxicity from ingestion which can cause central nervous system depression, which could result in death, permanent impairment, or necessitate medical or surgical intervention.   • Faribault Foods Inc., a Faribault, Minn. establishment, is recalling approximately 15,134 pounds of canned soup product due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The product contains milk and soy, known allergens, as well as beef and pork, which are not declared on the product label.

    The cans labeled as chicken noodle soup actually contain a meatball and pasta product. The canned soup items were produced on May 26, 2020. The following products are subject to recall: [View Labels (PDF only)]

    • 14-oz. cans of “PROGRESSO ORGANIC CHICKEN NOODLE SOUP” with a best by date of “BestByMAY262022” printed on the bottom of the can and a best by date of “09JUN2022” printed on the product case.

    The products subject to recall bear establishment number “EST18826A” printed on the bottom of the can under the best by date. These items were shipped to retail locations nationwide.

    The problem was discovered when the firm’s distributor notified FSIS of consumer complaints that the soup contained meatballs and pasta instead of chicken and noodles.

    There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an injury or illness should contact a healthcare provider.

    FSIS is concerned that some product may be in consumers’ pantries. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

Kader Exports did not distribute and have agreed to voluntarily destroy cooked shrimp found violative. The probable root cause for this recall is cross contamination. The company has eliminated the source of contamination and preventive steps were taken.

Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeriaem> infection can cause miscarriages and stillbirths among pregnant women.The affected products were sold in Wegmans stores in New Jersey, Pennsylvania, Virginia, North Carolina, Maryland, and Brooklyn and Harrison, NY.Affected produce items, sold between July 31 and August 7, 2020, include:

  • The U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) is issuing a public health alert because Bluegrass Provisions Co., a Crescent Springs, Ky. establishment, produced sausage products that may be contaminated with Listeria monocytogenes. A recall was not requested because it is believed that all products are no longer in commerce and are past their use or freeze by dates.

    The ready-to-eat smoked sausage items were produced on Apr. 22, 2020. The following products subject to the public health alert are: [View Label (PDF only)]

    • 14-oz. plastic packages containing 6 pieces of “BLUE GRASS METTWURST,” with a use or freeze by date of July 23, 2020.
    • 14-oz. plastic packages containing 6 pieces of “WALNUT CREEK FOODS Smoked Sausage,” with a use or freeze by date of July 23, 2020.
    • 14-oz. plastic packages containing 6 pieces of Lidl “SMOKED BRATWURST,” with a use or freeze by date of July 23, 2020.
    • 14-oz. plastic packages containing 6 pieces of Lidl “SMOKED BRATWURST WITH CHEESE,” with a use or freeze by date of July 23, 2020.

    The products bear establishment number “EST. 7417” inside the USDA mark of inspection. These items were shipped to distributors and retail locations in Kentucky, Ohio, North Carolina, and Virginia.

    The problem was discovered by routine testing and the results showed one of the products was contaminated with Listeria monocytogenes. The additional products may be affected by cross-contamination.

    Consumption of food contaminated with L. monocytogenes can cause listeriosis, a serious infection that primarily affects older adults, persons with weakened immune systems, and pregnant women and their newborns. Less commonly, persons outside these risk groups are affected.

    FSIS is concerned that some product may be in consumers’ freezers. Consumers who have purchased these products are urged not to consume them. These products should be thrown away or returned to the place of purchase.

    FSIS advises all consumers to reheat ready-to-eat product until steaming hot.

  • Ferring Pharmaceuticals US is voluntarily recalling all lots on the market of DDAVP® Nasal Spray 10 mcg/0.1mL, Desmopressin Acetate Nasal Spray 10 mcg/0.1mL, and STIMATE® Nasal Spray 1.5 mg/mL listed in the table below to the consumer level. These products are being recalled due to super potency or amounts of desmopressin higher than specified. These out of specification results were obtained during routine testing.The risks associated with higher than specified amounts of desmopressin relate to abnormally low levels of sodium in the blood (i.e., hyponatremia) which could eventually lead to seizure, coma, and death. To date, Ferring has not received an increase in adverse event reports due to increased concentrations of desmopressin from users of the nasal spray.   • Thomson International Inc. of Bakersfield, California is recalling Red, Yellow, White, and Sweet Yellow Onions shipped from May 1, 2020 through the present.  The onions are being recalled because they have the potential to be contaminated with Salmonella, an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.Onionswere distributed to wholesalers, restaurants, and retail stores in all 50 states, the District of Columbia and Canada.The onions were distributed in 5 lbs. carton. 10 lbs. carton. 25 lbs. carton. 40 lbs. carton, 50 lbs. carton. bulk, 2 lb. mesh sacks, and 3 lb. mesh sacks, 5 lb. mesh sacks, 10 lb. mesh sacks 25 lbs. mesh sacks, 50 lbs. mesh sacks under the brand names Thomson  Premium, TLC Thomson International, Tender Loving Care, El Competitor, Hartley’s Best, Onions 52, Majestic, Imperial Fresh, Kroger, Utah Onions and Food Lion.Consumers, restaurants, and retailers should not eat, sell, or serve red, white, yellow, or sweet onions from Thomson International, Inc. or products containing such onions. The products bear the establishment number “EST 5590,” “P5590” or “EST. 17417” printed on the packaging above the expiration date. These products were shipped to retail locations nationwide.

The problem was discovered by Ajinomoto Foods North America, Inc. when they identified pieces of hard plastic in their production process and in a barrel of diced green chilies that was received from their ingredients supplier, Sun Valley Foods. Sun Valley Foods initiated a recall of the green chilies with the FDA. As more FDA information becomes available, FSIS will update this public health alert.

There have been no confirmed reports of adverse reactions due to consumption of these products. Anyone concerned about an illness should contact a health care provider.
