  •  Conagra Brands, Inc., a Milton, Pa. establishment, is recalling approximately 32,400 pounds of canned beef products due to a potential processing defect, resulting in the potential survival of bacterial pathogens in the products, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.

    The products subject to recall bear establishment number “EST. 794” inside the USDA mark of inspection. These items were shipped to distribution centers and retail locations in Indiana, Kentucky, Maryland and Texas.

    The problem was discovered by the establishment on July 27, 2019 during routine activities.

    There have been no confirmed reports of adverse reactions due to consumption of these products.   • Home Market Foods, Inc., a Norwood, Mass. establishment, is recalling approximately 53,217 pounds of frozen ready-to-eat beef and pork meatball products due to misbranding and undeclared allergens, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. The products contain milk and wheat, known allergens, which are not declared on the product label. There have been no confirmed reports of adverse reactions due to consumption of these products.   • JuJu Bakes, LLC, DBA The Cookie Dough Cafe of Bloomington, IL is recalling 15 cases of 1.6 ounce Chocolate Chip Chilled Gourmet Edible Cookie Dough Bars, because they may contain peanuts. People who have an allergy or severe sensitivity to peanuts run the risk of serious or life-threatening allergic reaction if they consume these products.
    Product was distributed in IL, MI, PA, IN, KY, WI, OH, MN, WV via Lipari Foods and was distributed to select retail stores.
    The product comes in a 1.6 ounce, white and turquoise package marked with lot # 19177 with an expiration date of 01/22/20, UPC 850967006531. No other lots or expiration dates were affected.
    No illnesses have been reported to date.
    Allergen issue was found after an employee tasted peanut butter in a bar labeled chocolate chip. The employee sampling the item did not have a peanut allergy and did not experience a reaction. Her observation led to the recall of Lot #19177. Subsequent investigation indicates the problem was caused by a temporary breakdown in the company’s production and packaging processes.

  • Elevation Foods is voluntarily recalling containers of Archer Farms-brand egg salad; Freskëtbrand egg salad, tuna salad, and Thai lobster salad; and Archer Farms-brand deviled egg sandwiches produced on June 18, 2019 due to possible contamination with Listeria monocytogenes. Listeria monocytogenes is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain, and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women.
    We believe fewer than 1,087 cases of product have been directly shipped to retailer warehouses throughout the United States.
    No illnesses have been reported to date.
    The recalled products were manufactured at Elevation Foods’ Knoxville, Tennessee facility.
    Elevation Foods is working with distributors and retailers to quarantine and recover any impacted product remaining on store shelves.
    HOW TO IDENTIFY THE RECALLED PRODUCT:
    The containers have the “use by” dates stated below printed on the side of each container and the lot number stated for each product on the side or the lid (see attached pictures). This recall applies only to the products with the lot numbers and “use by” dates stated below.

    Elevation Foods identified the problem with the products after receiving positive test results for three containers of affected egg salad which were sampled and tested by the Florida Department of Agriculture and Consumer Services. Elevation Foods is continuing to investigate potential sources of the problem.

  • Mizkan America, Inc., announced the voluntary recall of select production codes of certain RAGÚ® pasta sauces in the U.S. because the sauce may contain fragments of plastic. There have not been any reports of consumer injuries or complaints. Mizkan America is taking this action out of an abundance of caution. This recall is at the retail level and all impacted retailer customers have been notified of this voluntary recall prior to this press release. Retail customers who have not been notified are not impacted by this voluntary recall. Mizkan America also asks consumers to examine their refrigerator and pantry inventory for the specific jars affected by this recall. Any recalled sauce should be discarded and not consumed.

  • Xeljanz, Xeljanz XR (tofacitinib): Drug Safety Communication – Due to an Increased Risk of Blood Clots and Death with Higher DoseFDA has approved new warnings about an increased risk of blood clots and of death with the 10 mg twice daily dose of Xeljanz, Xeljanz XR (tofacitinib), which is used in patients with ulcerative colitis. In addition, the approved use of tofacitinib for ulcerative colitis will be limited to certain patients who are not treated effectively or who experience severe side effects with certain other medicines. We approved these changes, including adding our most prominent Boxed Warning, after reviewing interim data from an ongoing safety clinical trial of tofacitinib in patients with rheumatoid arthritis (RA) that examined a lower and this higher dose of the medicine.Patients should tell your health care professionals if you have a history of blood clots or heart problems, and talk to them about any questions or concerns. Stop taking tofacitinib and seek emergency medical attention right away if you experience any unusual symptoms, including those that may signal a blood clot such as:• Sudden shortness of breath

We are recalling our products listed above as a precautionary action; to date there have been no consumer complaints or reported cases of Salmonellosis in connection with these products.
    Salmonella can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea, nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections, endocarditis and arthritis.

  • The Lennox Intl Inc located in Edison NJ, is voluntary recalling its Natural Pig ears because they have the potential to be contaminated with Salmonella. Salmonella can affect animals eating the product and there is risk to humans from handling contaminated products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products which are solely for the consumption by dogs.
    Healthy people infected with Salmonella should monitor themselves for some or all of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. Rarely, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.
    Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans. If your pet has consumed the recalled product and has these symptoms, please contact your veterinarian.

  • Premium Foods USA, Inc., a Woodside, N.Y. establishment and importer of record, is recalling approximately 76,025 pounds of various Siluriformes fish products that were not presented for import re-inspection into the United States, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today. Additionally, the products were imported from Bangladesh and Myanmar, countries ineligible to export Siluriformes fish product to the United States.
    The various Siluriformes fish items were imported from Bangladesh and Myanmar to the United States on various dates from March 26, 2018 through March 8, 2019. The spreadsheet contains a list of products subject to recall.
    These items were shipped to retail locations in Colorado, Connecticut, Florida, Georgia, Illinois, Michigan, New Jersey, New York and Pennsylvania.
    The problem was discovered during routine FSIS surveillance activities of imported products.

    The products subject to recall bear establishment number “EST. 21103” inside the USDA mark of inspection. These items were shipped to restaurants in Alabama, Florida, Georgia, Mississippi and Tennessee.
    The problem was discovered after the facility learned that an employee may have cut himself during production.

  • Salisbury, Maryland, Jubilant Cadista Pharmaceuticals Inc. is voluntarily recalling one lot of Drospirenone and Ethinyl Estradiol Tablets, USP, 3 mg/ 0.02mg, 28×3 Blister Pack/Carton to the consumer level. The affected product is being recalled due to out-of-specification (OOS) dissolution results at the 3-month stability time point. The affected product is manufactured by Cyndea Pharma, S.L., Olvega (Soria), 42110 Spain under contract from Jubilant Cadista Pharmaceuticals Inc., 207 Kiley Drive Salisbury, Maryland 21801.
    As a result of the OOS dissolution results, product efficacy may be decreased due to incomplete absorption of the active ingredients. To date, Jubilant Cadista Pharmaceuticals Inc. has not received any reports of adverse events related to this recall.
    Product Description: Drospirenone and Ethinyl Estradiol tablets are an estrogen/progestin combination oral contraceptive, indicated for use by women to:
    Prevent pregnancy
    Treat symptoms of premenstrual dysphoric disorder (PMDD) for women who choose to use an oral contraceptive for contraception
    Treat moderate acne for women at least 14 years old and/or if the patient desires an oral contraceptive for birth control
    Drospirenone and Ethinyl Estradiol tablets are packaged into a carton containing 3 blister cards. Each blister card contains 28-film coated, biconvex tablets, in the following order: 24 active pink-color round, unscored, film-coated tablets debossed with a “20” on one side, each containing 3 mg Drospirenone and 0.02 mg Ethinyl Estradiol, and four (4) inert white-color round, unscored, film-coated tablets debossed with a “PL”.
    The affected Drospirenone and Ethinyl Estradiol Tablets, USP, has been identified as Lot number 183222, with NDC number 59746-763-43 and expiration date of 11/2020.
    Drospirenone and Ethinyl Estradiol Tablets, USP, 3 mg/0.02 mg, lot 183222 was distributed Nationwide to wholesalers, distributors, and retailers.
    Jubilant Cadista Pharmaceuticals Inc. is notifying its customers by emailing a recall notification letter and response form and is arranging for return of all recalled product.
    Patients that have used the affected lot of Drospirenone and Ethinyl Estradiol Tablets, USP, 3 mg/0.02 mg should consult their healthcare provider. Patients may return the affected lot to their place of purchase.
    Wholesalers, distributors, and retailers should immediately examine their inventory for the affected lot. All inventory of the affected lot should be quarantined to prevent further distribution to patients. Customers who purchased the impacted product directly from Jubilant Cadista Pharmaceuticals Inc. can call Inmar at 1-855-205-9246 (9:00 a.m. – 5:00 p.m. EDT, Monday – Friday) to arrange for their return.

    The problem was discovered when FSIS inspection program personnel determined that the firm had received a consumer complaint reporting that the product was incorrectly labeled.

Allergan is taking this action as a precaution following notification of recently updated global safety information concerning the uncommon incidence of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) provided by the U.S. Food and Drug Administration (FDA).BIOCELL®saline-filled and silicone-filled textured breast implants and tissue expanders will no longer be distributed or sold in any market where they are currently available. Effective immediately, healthcare providers should no longer implant new BIOCELL® textured breast implants and tissue expanders and unused products should be returned to Allergan. Allergan will provide additional information to customers about how to return unused products.Patient safety is a priority for Allergan.   • Mountain Rose Herbs (MRH) of Eugene, Oregon is recalling all sizes ranging from 4 oz. to 50 lbs. of its Fennel Seed Whole, Lot #25031, because they have the potential to be contaminated with Salmonella, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy persons infected with Salmonella often experience fever, diarrhea (which may be bloody), nausea, vomiting and abdominal pain. In rare circumstances, infection with Salmonella can result in the organism getting into the bloodstream and producing more severe illnesses such as arterial infections (i.e., infected aneurysms), endocarditis and arthritis.
    The recalled Fennel Seed Whole was sold directly to customers who ordered from mountainroseherbs.com, purchased at the company’s retail shop in Eugene, Oregon, or placed mail orders by phone or fax. The affected product was also distributed to vitamin and health food stores in AZ, CA, CO, FL, IL, MA, MI, MN, MT, NC, NE, NM, NY, OH, OR, PA, SC, TN, TX, UT, VA, VT, WA, WV and to one consignee in Canada. It is possible MRH customers also resold the Fennel Seed Whole to their own clientele.
    The product comes in a clear plastic or poly-woven bag marked with lot # 25031, which is located on the principal display panel on the lower left of the label. Product has been sold and distributed between April 2019 and up to July 18, 2019.
    No illnesses have been reported to date.
    The potential for contamination was noted after testing by the company revealed the presence of Salmonella in a portion of its active inventory. Production of the product has been suspended while MRH continues our investigation.

  • Northfork Bison Distributions Inc. of St. Leonard, Quebec is recalling its Bison Burgers & Bison Ground because they have the potential to be contaminated with E. coli: O121 and O103. This form of E. coli may cause diarrheal illness often with bloody stools. Although most healthy adults can recover completely within a week, some people can develop a form of kidney failure called Hemolytic Uremic Syndrome (HUS). HUS is most likely to occur in young children and the elderly. The condition can lead to serious damage and even death.
    The recalled Bison Burgers & Bison Ground were distributed across Eastern & Central U. S., to institutions and retail outlets.
    The institutions that have Bison Burgers and Bison Ground in their inventory with the production date: February 22, 2019 – April 30, 2019, are urged to contact their distributor for a full refund. For the retail outlets, the Bison Burgers (4 x 4 oz) in question, with the expiration date up to October 8th, 2020, can be clearly identified by their blue boxes with the Northfork Bison logo prominently displayed.   • Growers Express takes issues of food safety seriously and in an abundance of caution and in the interest of protecting consumers is expanding the list of select fresh vegetable products affected by a voluntary recall that was issued on July 1, 2019. After further testing of an additional product lot from the suspected source of the recall, a single retail sample was found to be positive for Listeria monocytogenes. The company has expanded the list of recalled products to include certain specific fresh Brussels sprouts, fresh cauliflower florets and fresh green beans products with the “best by” or “pack dates” identified in the chart below.
    The products originated from a Growers Express production facility in Biddeford, Maine and were distributed to the states of Massachusetts and Maine. Growers Express is no longer sourcing vegetables from the suspected cause of the contamination. There are no reported illnesses to date.
    Listeria is an organism that can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer short-term symptoms such as high fever, severe headaches, stiffness, nausea, abdominal pain and diarrhea. Listeria infection can cause miscarriages and stillbirths among pregnant women.

  • Pita Pal Foods, LP of Houston, TX has issued a voluntary recall of certain hummus products made between May 30, 2019 and June 25, 2019, due to concerns over Listeria monocytogenes, which was identified at the manufacturing facility (not in finished product) during an FDA inspection.

  • Northfork Bison Distributions Inc. of St. Leonard, Quebec is recalling its Bison Burgers & Bison Ground because they have the potential to be contaminated with E. coli: O121 and O103. This form of E. coli may cause diarrheal illness often with bloody stools. Although most healthy adults can recover completely within a week, some people can develop a form of kidney failure called Hemolytic Uremic Syndrome (HUS). HUS is most likely to occur in young children and the elderly. The condition can lead to serious damage and even death.
    The recalled Bison Burgers & Bison Ground were distributed across Eastern & Central U. S., to institutions and retail outlets.
    The institutions that have Bison Burgers and Bison Ground in their inventory with the production date: February 22, 2019 – April 30, 2019, are urged to contact their distributor for a full refund. For the retail outlets, the Bison Burgers (4 x 4 oz) in question, with the expiration date up to October 8th, 2020, can be clearly identified by their blue boxes with the Northfork Bison logo prominently displayed. Consumers who have purchased these Bison Burgers are urged to return them to the place of purchase for a full refund.

  • Giraffe Infant Warmers and Panda i-Res Infant Warmers by GE Healthcare: Recall – Due to Bedside Panels and Latch Areas Cracking or Breaking
    Customers with questions or concerns regarding this recall may contact GE Healthcare Service at 1-800-437-1171 or they can call their local Service Representative.GE Healthcare is recalling the Giraffe and Panda i-Res Infant Warmers because the bedside panels and latch areas of the warmers can crack or break if the unit is moved using the bedside panels instead of the front or rear maneuvering handle (see figures below). If an infant comes in contact with a bedside panel with a cracked or broken latch, the panel can disengage and fall open, no longer protecting the infant from falling from the warmer.The firm has received a total of 338 complaints related to bedside panels/latches. This includes two infants who fell from warmers and fractured their skulls. There were no deaths reported.
  • Fairmont Foods, Inc., a Fairmont, Minn. establishment, is recalling approximately 35,145 pounds of ready-to-eat pork and beef gravy products that may be contaminated with extraneous materials, specifically clear pliable plastic, the U.S. Department of Agriculture’s Food Safety and Inspection Service (FSIS) announced today.
    The frozen, ready-to-eat pork and beef gravy items were produced on May 2, 2019. The following products are subject to recall: [View labels (PDF only)]
    45-lb. cases containing nine 5-lb plastic bags of “Popeyes LOUISIANA KITCHEN PORK TASSO AND GROUND BEEF GRAVY” with Product Code 1F0112, Use By: 050220, and a time stamp ranging from 1615 to 0022.
    The products subject to recall bear establishment number “EST. 2WM” inside the USDA mark of inspection. These items were shipped to Popeyes Louisiana Kitchen restaurant locations in Louisiana.
    The firm contacted FSIS after they received a report that a consumer found a piece of the soft pliable plastic in the product.
    There have been no confirmed reports of adverse reactions due to consumption of these products. FSIS has received no additional reports of injury or illness from consumption of these products.   • With the recent announcements of the Food and Drug Administration (FDA) and the Centers for Disease Control and Prevention (CDC) investigating contaminated Pig Ear Treats connecting to Salmonella, Pet Supplies Plus is advising consumers it is recalling bulk pig ear product supplied to all locations by several different vendors due to the potential of Salmonella contamination. Salmonella can affect animals eating the products and there is risk to humans from handling contaminated pet products, especially if they have not thoroughly washed their hands after having contact with the products or any surfaces exposed to these products.Individuals infected with Salmonella should monitor for some, or all, of the following symptoms: nausea, vomiting, diarrhea or bloody diarrhea, abdominal cramping and fever. Rarely, Salmonella can result in more serious ailments, including arterial infections, endocarditis, arthritis, muscle pain, eye irritation, and urinary tract symptoms. Consumers exhibiting these signs after having contact with this product should contact their healthcare providers.Pets with Salmonella infections may be lethargic and have diarrhea or bloody diarrhea, fever, and vomiting. Some pets will have only decreased appetite, fever and abdominal pain. Infected but otherwise healthy pets can be carriers and infect other animals or humans.     Products containing glass particulate should not be administered intravenously due to the potential for life-threatening consequences. Reports in the literature suggest that sequelae of thromboembolism, such as pulmonary emboli, phlebitis, granulomas, or fibrosis may occur.
    To date, Fresenius Kabi has not received any complaints or reports of adverse events related to this recall.The company is issuing this notification after finding glass particulate in five vials in retained sample inventory of lot 6120341 during an inspection for a quality investigation. The second lot (6120420) is included in the recall as a precautionary measure as it was produced in the same filling campaign.

  • Growers Express takes issues of food safety seriously and is issuing a voluntary recall of select fresh vegetable products in an abundance of caution and in the interest of protecting its customers and end consumers. The fresh vegetable products include packaged varieties of butternut squash, cauliflower, zucchini and a butternut squashed based veggie bowl.     The products originate from a Growers Express production facility in Biddeford, Maine and were distributed to the states referenced in the chart below.     The Aqueon Betta Food, as referenced above and the subject of this recall, is sold only in a 0.95 oz plastic jar with the expiration dates of EX04JUN22 and EX05JUN22, item number 100106051, UPC Code 0 15905 06051 6, and manufacturing dates of 06/04/2019 and 06/05, 2019 (see pictures for easier identification).
    No illnesses have been reported to-date.
